CHURCH WEDDING
- a photocopy of a valid passport or ID
- baptism certificate

- certificate of completing a pre-marriage course
- permission from the parish priest to enter into marriage in a different parish
FOR CONCORDAT WEDDING, IN ADDITION TO THE ABOVE
- ID with address
- full birth certificate, issued no sooner than 3 months before the planned wedding date (90 days)
- confirmation of marital status issued by the appropriate civil registrar no sooner than 3 months before the planned wedding date (90 days)
- a certificate confirming that it is permitted for the person to enter into marriage abroad according to the local law
CIVIL MARRIAGE
- photocopy of a valid passport or ID, or other identification document containing address
- full birth certificate, issued no sooner than 3 months before the planned wedding date (90 days)
- potwierdzenie o stanie wolnym wydane przez polski USC nie wcześniej niż 3 miesiące przed planowaną datą ślubu (90 dni)
- a certificate confirming that it is permitted for the person to enter into marriage abroad according to the local law
